Health Insurance Through the Dominican Social Security System: A Lesson Every Dominican Worker Should Know

Every month, when we receive our paycheck, we notice a deduction for Social Security. Many employees accept it without asking questions, while others believe it only covers health insurance. In reality, these contributions finance several fundamental pillars of social protection in the Dominican Republic.
As an insurance advisor with extensive knowledge of the Dominican insurance system, I frequently meet people who are unaware of the benefits they receive through their Social Security contributions. My first recommendation is always the same: before purchasing additional health insurance, understand the coverage you already have and learn how to make the most of it.
What Happens When You Start a New Job?
Let's look at a practical example. Suppose today is July 9, 2026, and you sign an employment contract with a company. During your first payroll period, your employer submits your mandatory Social Security contributions through the Dominican Social Security Treasury (TSS).

However, many people believe their health insurance becomes active immediately. This is not the case. Generally, Family Health Insurance (Seguro Familiar de Salud - SFS) becomes effective approximately 45 days after the enrollment process is completed and the first contributions have been successfully processed through the system. During this period, the TSS validates the employee's information, and the Dominican Social Security System completes the enrollment process.
Who Chooses My Health Insurance Administrator (ARS)?
This is one of the questions I receive most often. The answer surprises many people. Neither you nor the company you work for initially selects your Health Risk Administrator (Administradora de Riesgos de Salud – ARS).

When an employee enters the Dominican Social Security System for the first time, the system itself automatically assigns the employee to an ARS according to the procedures established by law. For example, you may initially be assigned to:
ARS Universal
Primera ARS Humano
Mapfre Salud ARS
ARS Futuro
ARS Monumental
ARS Yunén
ARS Simag
ARS Renacer
SeNaSa (National Health Insurance)
ARS CMD
ARS UASD
ARS National Police
ARS Armed Forces
...among other Health Risk Administrators authorized to operate within the Dominican Social Security System. It is important to understand that the company you work for does not control this initial assignment, as it is determined by the Social Security System itself.
Can I Change My ARS?
Yes. Once you have completed twelve (12) consecutive months of contributions to the Dominican Social Security System through formal employment, with all contributions properly reported through the Dominican Social Security Treasury (TSS), you become eligible to transfer to the ARS of your choice.
The process is very simple. You only need to visit the ARS you wish to join and formally express your desire to transfer. The new ARS will manage the entire process in accordance with the regulations established by the Superintendency of Health and Occupational Risks (SISALRIL). Therefore, if you were assigned to an ARS that was not your preferred choice during your first year, it does not mean you must remain there permanently. After completing the required twelve months of continuous contributions, you are free to select the ARS that best meets your healthcare needs.
Who Pays for Social Security?
Another common question is who actually pays for health insurance. The Dominican Social Security System is financed jointly by the employer (the company or individual you work for) and the employee (you, the person employed by that company). Both make monthly contributions that finance the benefits established under Law No. 87-01, which created the Dominican Social Security System.

Notice that the deduction you see for Family Health Insurance represents only a portion of the actual cost of your healthcare coverage. While you contribute 3.04% of your salary, your employer contributes an additional 7.09%, jointly financing your medical protection.
What Does Family Health Insurance Actually Cover?
Family Health Insurance provides coverage for:
Medical consultations.
Emergency care.
Hospitalization.
Surgical procedures.
Clinical laboratory services.
Diagnostic testing.
Outpatient prescription medications.
Pregnancy, childbirth, and postpartum care.
Preventive healthcare programs.
Benefits included in the Dominican Health Services Plan (PDSS).

However, it is important to understand that having health insurance does not mean that every medical service is covered. Each benefit is subject to coverage limits, prior authorizations, co-payments, and exclusions established by current regulations.
Why Do Many People Purchase Supplemental Health Insurance?
Based on my experience advising families and businesses, I can confidently say that Family Health Insurance provides an excellent foundation of protection. However, it may not fully satisfy every individual's healthcare needs.
Supplemental health insurance plans allow policyholders to:
Increase coverage limits.
Reduce co-payments.
Access a broader network of healthcare providers.
Obtain private hospital accommodations.
Receive international medical coverage.
Access additional benefits not included in the basic Social Security plan.

Supplemental insurance does not replace Social Security: it enhances it.

Understanding how the Dominican Social Security System works allows individuals to make better financial and healthcare decisions while maximizing the benefits for which both they and their employers contribute every month. Insurance education begins with understanding how your rights are established within the system, when your coverage becomes effective, how your ARS is assigned, and when you gain the right to choose the healthcare administrator that best meets your needs. Because an informed employee not only makes better use of health insurance but also protects their own well-being and that of their family.
